Reablement Support Workers visit people in their own homes. Often, we are the first ones there when someone comes home from hospital, welcoming them home and providing essential help and support to help them settle back in.
Reablement is not the same as Home Care. We help people set their own goals, and help them achieve them; from making tea to controlling their own medication, Reablement Support Workers work with each person to help them keep their independence.
Calls are not time limited; you will spend as much time with each person as they need (in agreement with your Team Leader) – allowing real success to be built in each persons recovery. Reablement isn’t endless though; usually we spend between 6 and 8 weeks with each person, so you will also learn to say fond goodbyes before helping someone else.
"Would you like to say that all the Carers have been fantastic, very caring with D. In fact we are both very sad that we wont be seeing them again. Keep the good work up ladies and thank you all from the bottom of our hearts." -DB
"I have been extremely well looked after since I left Hospital. All Libertas staff have been professional, helpful and very friendly, they have all aided my recovery and I am more able and confident as a result of their support and care. Thank you." -AM
Starting with a full and comprehensive training package, you will learn about Reablement and the differences from traditional Home Care. You will then go on to a fully supported shadowing and mentoring programme, helping you meet people in the community and provide help and support alongside and experienced Reablement Support Worker. You will be attached to one of our 12 teams in Lincolnshire, providing help and support to people in your local zone or area with the support and guidance of an experienced Team Leader.
Working in your local community you will visit people in their own homes, providing essential and hugely valuable help and support.
Duties can vary widely; the people we help set their own goals and we help them achieve them; from making tea or tackling the stairs to learning a different way to shave or use a new mobility aid. Every episode of Reablement is different, and you will learn new things every day.
The support you give enables people to learn new skills and new ways of doing things, things that allow people to live a more independent, more fulfilled life – at home. It is a hugely rewarding job.
We welcome applications from candidates with any of the following skills or attributes: Care, Support, Care Worker, Social Worker, Support Worker, Support Living, Social Care, Healthcare, Home, Care, Homecare, Reablement, Recovery

Visits are usually in different places so the ability to travel, usually by car, is essential.
Experience and qualification can all help and get your career with Libertas off to a flying start. More importantly patience, understanding and empathy are essential, and this is what we look for in our applications. Can you sit and talk to someone? Can you help someone do something while still making them feel valued and respected?
A background in Health and Social Care, or experience of caring for someone, may be an advantage. We are however, far more interested in you as a person and what you can bring to the role. Many people who have never considered Caring as a Career, or who are new to the job, find that this unique, exceptional and incredible valuable role is hugely rewarding.
